About the role
Hospital in the Home (HITH): Provides acute hospital-level care to patients in their own homes as a safe alternative to inpatient admission.
- Practice in line with NSW Health guidelines, local policies, and protocols.
- Deliver evidence-based nursing care to people of all ages and cultural backgrounds, including individuals, families, and communities.
- Assess, plan, implement, and evaluate care collaboratively to achieve health goals and optimal outcomes.
- Lead and coordinate nursing and healthcare across settings, including referrals and collaboration with other professionals and services.
- Practice independently and collaboratively, maintaining accountability and appropriately delegate care based on staff education, training, and context.
About you
- Currently registered as a Registered Nurse (division 1) with the Australian Health Professionals Regulation Agency (AHPRA).
- Current NSW unrestricted driver's license (class c or above) in order to attend client's home with provided car.
- Experience working within community healthcare settings is highly desired.
- Demonstrated ability to work within an interdisciplinary team using effective communication and interpersonal skills.
- Demonstrated effective time management skills and ability to prioritise competing workloads within a complex environment.
