Job Description

RESPONSIBLE FOR PLANNING, DEVELOPMENT AND FOLLOW-UP OF AN APPROPRIATE PLAN OF CARE FOR PATIENTS IN NEED OF EXTENDED CARE PLACEMENT OR HOME CARE FOLLOWING DISCHARGE FROM THE INPATIENT REHAB FACILITY.

RESPONSIBLE FOR PROVIDING SUPPORTIVE PSYCHO-SOCIAL ASSISTANCE FOR APPROPRIATE PATIENTS/FAMILIES.

RESPONSIBLE FOR PROVIDING SHORT TERM COUNSELING AS NEEDED AND MAKING APPROPRIATE REFERRALS.

 

 

Additional Job Description

Education: Master's degree in a social work or social welfare program accredited by the Council on Social Work Education. Credentials: MSW licensure by the South Carolina Board of Social Work Examiners.  Certification in case management preferred.
