RESPONSIBILITIES AND DUTIES Ensure compliance with all aspects of the 340b program Provide direction and oversight to the 340b technicians Implement all applicable aspects of the Office of Pharmacy Affairs guidance as well as OhioHealth policy and procedure Review of all internal 340b audits performed by 340b technicians and address any performance gaps discovered by the audits Serve as the Covered Entity Primary Contact Provide ongoing review and maintenance of the HRSA database Complete and submit annual Medicare Cost Report information Coordinate and prepare for any external 340b audit Develop response and action plan to any internal or external 340b audit findings Own educational efforts around 340b program Work with necessary affiliated departments / internal and external stakeholders, acting as the liaison to ensure compliance of the program Evaluate optimization and expansion opportunities of the 340b program Performs other compliance / auditing functions as assigned 40% Clinical and Operational Activities The individual will focus on the clinical workload and operational staffing for the day. Specific activities include: realignment of resources to meet daily workload, re-assignment of staff due to call-offs, clinical services outcomes results and oversight of clinical services program. Reports to be generated as requested by department leadership to quantify and qualify the clinical and operational activities of the department. Other specific areas of focus include formulary and purchasing management. 40% Clinical Quality and Regulatory Oversight The individual must be prepared to oversee the quality and safety plan of the department and work collaboratively with others within the organization to address safety concerns. Activities may include general report analysis, root cause analysis facilitation meetings or trending the safety events within the organization. In addition, the individual must be aware of all regulatory requirements for pharmacy including but not limited to The Joint Commission, Board of Pharmacy, USP, EPA, and DE a . He/She will work in conjunction with the department leadership to ensure compliance within the entire organization. 10% : Committee Facilitation and Organizational Alignment Committee involvement is extensive and in many cases the individuals will chair the committee. Committees include: Medication Management, Pharmacy and Therapeutics, Formulary Sub-Committee, Medication Safety and The Joint Commission Core group. This position will be required work across the system for ultimate success. 10% : Maintenance of Clinical & Operational Skill Sets The individual will continue to have direct patient care not necessarily in contact with the patient t to maintenance competence. Job Description Summary: The Oncology Clinical Pharmacist has a focused approach in working with OhioHealth’s multi-disciplinary oncology care teams. Primary responsibilities include: • Promoting evidence and value-based oncology & supportive medication therapy through close participation in the oncology medication therapy decision-making process with the multi-disciplinary care team. • Recommending modifications and improvements in oncology & supportive medication therapy based on information obtained through patient interviews, objective monitoring parameters and the patient medical record. • Modifying chemotherapy plans in the electronic health record (EHR), in collaboration with the multi-disciplinary team, to ensure plans are accurate for patient’s age and performance status, pre-medications are appropriate for patient-specific factors and relevant drug interactions have been identified and addressed. • Communication and documentation of findings and pharmacist interventions with providers and providing appropriate alternatives to current treatment plans as needed. • Position may include optimization of patient medication adherence through counseling, education, and application of motivational interviewing skills. Documentation of patient education at therapy initiation and follow-up, including review of oncology & supportive medication effectiveness, side effects, lab monitoring, expected outcomes of therapy. • Understanding of Specialty Pharmacy Benefit Managers (PBMs) and formulary requirements. Identifying and recommending alternative medications for requests for non-formulary specialty drugs required by insurances. • Providing experiential instruction to Doctor of Pharmacy Students and OhioHealth pharmacy residents.
