Position Title: Health Service Manager – The Way Back Service/Towards Zero Suicide
Employment Type: Full Time, Maximum Term (until June 2026)
Hours per week: 38 hours
Position Grade: Health Service Manager Level 3
Salary: $122,850.00 to $139,559.00 base per annum

About the role:

The Aftercare Lead (the Lead) provides leadership for the integration of aftercare services including The Way Back NSW Aftercare Service in SVHA. The Way Back NSW provides community-based assertive psychosocial support for people following a suicide attempt or suicidal crisis who have been referred from an acute care or community mental health setting.

This role will lead the coordination of Aftercare services with relevant hospital and community mental health and suicide prevention services to ensure access to aftercare support is available for all people who need it.

The Lead will be required to plan and implement strategies to enhance service visibility for patients/consumers, families and clinicians, coordinate with teams to ensure the effectiveness of referral pathways, and play a lead role in clinical and operational governance consistent with NSW Health and SVHA policies, procedures and standards.

The positon will manage a range of direct reports, including nursing, social worker, occupational therapists, clinical psychologists, psychologists and mental health peer workers. The manager will work closely with the staff specialists of each respective team to further service excellence and delivery.

A key imperative of this role will be to establish a model which provides alternatives to attending the emergency department e.g. A Safe Haven Café. The role would expand upon the community mental health peer work force and explore service delivery which will lead to improved mental health outcomes for the consumers of the SVH catchment area and beyond. The manager will be a central positon in clinically and operationally leading the development of these models through co-design and in reporting and evaluating the findings both a local and ministry level.

Selection Criteria:
1. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia
2. Relevant tertiary qualification in a health related or community development field and or appropriate health experience including relevant work experience in mental health

3. Demonstrated knowledge of strengths based, recovery oriented and trauma informed approaches with a focus on suicidal crisis and the ability to link guests to appropriate services.

4. Proven experience in complex and multi-site project management and change management in a health setting, preferably in mental health and suicide prevention.

5. An applied understanding of contemporary approaches to suicide prevention and service delivery models including the application of Clinical Care Standards, outcome measurement, evaluation and Value Based Health Care
6. Demonstrated ability to work collaboratively with senior health managers, clinical leaders, research partners and other external stakeholders. This includes facilitation and negotiation skills such as the ability to resolve conflicts, manage competing demands and prioritising resources.

7. Ability to provide in-depth advice and education to enhance service delivery and to guide and support  others in providing service excellence.

8. Knowledge of clinical information systems, including those pertaining to workflows, data flow, change processes, testing activities, data extraction and reporting; and the implications of such systems on the development and application of IT solutions for performance monitoring and continuous improvement
