About the Role

An exciting opportunity has become available at St. Vincent's Hospital Melbourne in our Hospital in the Home Service. This position is for a HITH Registered Nurse. HITH has two shifts per day, seven days per week and is supported by a medically-led, multidisciplinary team. This has grown from 15 beds to a minimum bed occupancy of 30 beds. HITH sits under the umbrella of SV@H, alongside our partners – District Nursing. HITH specialises in the delivery of acute-level care via a bed-substitution and emergency department-diversion model (this includes direct care and telehealth for patients requiring COVID treatment and management). HITH has its own very busy nurse-led clinic and provides nursing direction to The Cottage, our special, 5-bed, on-site accommodation for patients experiencing or at risk of experiencing homelessness. Geographically-diverse, HITH spans a 20km radius with visits to 30km when needed and subcontracting beyond 30km and into regional. We are looking for smart, motivated nurses with a can-do attitude to join our team!

Classifications for this position will be YW15 ​($107,962.40 pro rata per annum)


Your Contribution

  • Bachelor of Nursing

  • Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)

  • A current Victorian Driver’s License

  • Minimum of 3-4 years post-graduate experience

  • Relevant experience within the community setting and/or across a broad range of acute services

  • Demonstrated advanced clinical skills and knowledge of best practice in clinical nursing practice

Working in Hospital in the Home

St Vincent’s at Home (SVAH) is a multi-disciplinary team providing Hospital in the Home (HITH) and community based nursing services. SVAH also provides HITH nursing care to The Cottage, a six -bed short stay facility for people experiencing homelessness. HITH is an acute bed substitution model that provides the equivalent of acute care within a patient’s home. The multidisciplinary team of medical, nursing and allied health staff enable earlier supported transfer from hospital and diversion from the Emergency Department where possible. The position may be based at any of the St Vincent’s Hospital Melbourne sites and requires staff to undertake home visiting duties.
