About the Role

Based at our Hawthorn Clinic, this is a great opportunity to deliver senior case management in community mental health. You will provide a high standard of clinical assessment, treatment and support to clients of the St. Vincent’s integrated adult MHS and their carers, demonstrating excellence in discipline specific and recovery-focused case management skills.

You will join a dynamic community mental health team operating under a recovery-oriented model of care, utilising the Strengths Model of Case Management.

  • Build a therapeutic alliance with the consumer and family/carers and actively engage them in the planning, delivery and evaluation of their care
  • Undertake the assessment, provision of treatment and delivery of targeted interventions to consumers with complex mental health related needs and their family/carers
  • Assist in the provision of primary, secondary and tertiary consultation services, community development and education, liaison and linkage to other agencies
  • Work within the Strengths Model of recovery orientated practice
  • Act as a role model for your junior colleagues to ensure ongoing provision of excellent standards of care and development.

Classifications for this position will range from:

  • RPN Grade 4 NP75 - NP77 ($110,801 - $119,324 per annum)

  • OT/SW Grade 3 YB24 – YB27 or YC46 - YC49 ($99,606 - $113,796 per annum)

  • Psychologist Grade 3 PL1 – PL4 ($104,078 - $119,095 per annum)


Your Contribution

  • Current registration with AHPRA (if applicable)
  • A minimum of 5 years (7 years for Allied Health) post qualification clinical experience within Mental Health Services
  • Extensive experience in the provision of assessment, discipline specific intervention and case management services for consumers with serious mental illness and associated high level of disability
  • Experience in providing clinical supervision to other mental health staff
  • A comprehensive understanding and working knowledge of the Mental Health and wellbeing Act 2022 (including amendments) and other relevant legislation
  • Current Victorian driver's license
  • Current National criminal history check, or willing to obtain

About Hawthorn Community Mental Health

St. Vincent’s Mental Health (SVMH) includes an adult area mental health service for the inner city areas of Yarra and Boroondara. 

The Hawthorn Continuing Care Teams provide ongoing specialist treatment and support to meet the identified needs of clients who suffer with a serious mental illness and associated high level of disability in the City of Boroondara. The multidisciplinary team provides a range of clinical and therapeutic interventions using the recovery-focused Strengths Model of Recovery-Oriented Practice.
