ABOUT YOUR NEW ROLE:
We have an available PART TIME REGISTERED NURSE – CANA INCLUSIVE HEALTH PROJECT
The purpose of the role of the Registered Nurse – Cana Inclusive Health Project is to ensure the development, implementation and delivery of a pilot primary health service for members of Cana Communities in Redfern/ Waterloo. The primary responsibilities of the role will be the initial scoping of the acute and chronic health needs of members of Cana Communities and the delivery of acute primary nursing assessment and care where indicated. This position will also assist in the facilitation of referrals and support for people to access additional health services as required and the development of pathways to streamline service access. The Registered Nurse - Cana Inclusive Health Project will also develop and facilitate health education workshops and programs for the members of Cana Communities.

ABOUT YOU:
The candidate must have:
Current AHPRA Registration
Bachelor’s Degree in Nursing
Completion of basic life support training/CPR/First Aid
Minimum of 5 years’ post-registration experience
Demonstrated capacity to engage with clients with complex psychosocial and health needs, including through an outreach based model of care
Ability to work autonomously and within a team setting
Ability to demonstrate clinical knowledge in the specific area of practice with a commitment to professional development
Proactive involvement in patient health/disease management through care planning, case conferencing and health assessments
Excellent skills in oral and written communication and strong interpersonal skills
Excellent communication skills
Time management skills
Computer literacy
Desirable
Experience in the delivery of acute and sub-acute health care to marginalised clients including those experiencing mental health and/or substance abuse issues
Demonstrated Leadership skills
