Job Description:

An exciting opportunity currently exists for a Senior Registered Nurse to join our small friendly Community Care Team. The purpose of this position is to coordinate and attend to clients as a Registered Nurse as part of their community package.

As a Registered Nurse, you will play a pivotal role in ensuring a high standard of care is maintained at all times. You will be part of a small friendly and dedicate team of Home Care employees and provide Nursing assistance within the Community.

As part of St Vincent’s Community Services team, this position includes but is not limited to the following:-

  • Will provide advanced level of clinical case management and coordination of clinical service provision to clients;
  • Undertake all clinical assessments and clinical care plans.
  • Will work collaboratively with the Community Co-ordinators and State Manager for the effective management of the Community Care programs; and
  • Shared responsibility for the achievement and maintenance of quality improvement activities, including the Community Service quality reporting Management and monitoring of the Community Home Care Employees within the workplace including the completion of work performance appraisals; and
  • Attend to clients in a Registered Nurse capacity for those client whom have Nursing care as part of the community package; and
  • Compliance with all legislative and organisational reporting requirements.

Successful applicants will possess the following:-

  • Current AHPRA registration; and
  • Ideally three (3) years experience in a leadership role ; and
  • National Criminal History Check or the willingness to apply for one; and
  • Current and valid Open Driver's Licence.
