ABOUT THE POSITION:
Join the St Vincent’s Private Hospital Brisbane team as a Permanent Part time or Casual Social Worker and you will be working across our spectacular Kangaroo Point location. You will be pivotal in assisting our clinical staff and our patients during their stay with us.

ABOUT THE ROLE:

We are looking to speak with a Social Worker who is currently open to new opportunities to join our Allied Health team on a Permanent Part time or Casual basis. 

You will be an integral member of a multi-disciplinary health team, with demonstrated experience within a hospital setting:

• Conduct assessments of patients’ medical history, current health condition and treatment goals. 
• Coordinate with our team of specialists and clinical staff to ensure that our patient’s needs are being met.
• Guide our patients through transitions of returning home following procedures.

ABOUT YOU:

Experience in providing a service that enables patients and their families to deal with the impact changes in health can have on individual and family functioning.

• To ensure Social Work referrals are triaged and accessed in a timely manner and support the achievement of care plans.
• To ensure prompt assessment of newly referred patients
• To work closely with the multidisciplinary team involved in the care of patients and their families.
• To manage and escalate client issues or complaints appropriately.
• Contribute to development to of clinical practices, procedures and protocols within the Social Work department with the support of senior staff.
• Demonstrated commitment to professional excellence.
• Experience in Palliative Care services highly regarded.
• Solid working knowledge of community services, referral pathways and Hospital systems essential
• 2 – 5 years’ experience within a hospital setting including NDIS and My Aged Care processes will be highly regarded.

REMUNERATION:
The position of Social Worker, in addition to our competitive salary packaging options, is a Classification/Grade with a base remuneration of $41.17-$58.26 + superannuation based on experience level and relevant qualifications, exclusive of shift penalties or allowances.
