About the Role
You will work with clients living with complex healthcare needs, providing assessment and intervention to achieve optimal functional recovery. You will encourage self-management and empower clients to improve their own health outcomes, wellbeing and quality of life

Early identification of ALERT patients in the Emergency Department

Direct clinical provision of high quality and evidence-based Occupational Therapy specific assessment, intervention, education and management

Undertake specialist assessment of the person, the environment and occupational performance for clients with complex and chronic conditions, incorporating self-management approaches

Collaborative care planning with clients, carers and/or families in the context of their health, functional, psychosocial, cultural, environmental and economic needs


Classifications for this position will range from VF6 – VF9 ($86,730 - $99,699 per annum).

Your Contribution
Current registration with AHPRA as an Occupational Therapist
• Minimum 2 years’ experience in relevant acute/community setting
• Experience delivering services in the community, addressing the needs of clients living with chronic diseases and/or complex health needs and psychosocial conditions

• Ability to perform comprehensive assessments and develop collaborative plans that address physical, psychosocial and functional aspects of the client

• Current National criminal history check

About ALERT

This position is based at St Vincent’s Hospital Melbourne within the Emergency Department and also provides support and intervention in the community as part of an interdisciplinary team.

Key target groups include patients with complex needs who are impacted by one or more of the following key risk factors: substance use, mental health issues, homelessness, disability, complex aged care issues, and family violence.
