Job Description:

Occupational Therapist Grade 2 - Community Rehabilitation

15 hours/ week, Permanent position

About the Role
An exciting opportunity currently exists for a Grade 2 Occupational Therapist to join the Community Rehabilitation team. The Grade 2 Occupational Therapist will deliver high quality person-centred, goal-orientated, home and centre-based community rehabilitation within a supportive multidisciplinary team. The position may be required to work within any of the HIP programs across the HIP sites.
 

The classification range for this position will be VF6 - VF9 ($86,730 - $99,699 per annum, pro rata).
 

Your Contribution

  • Bachelor of Occupational Therapy or equivalent

  • Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA)

  • Minimum of four (4) years post –graduate experience in a relevant field of rehabilitation and or community based service

  • Demonstrated Occupational Therapy clinical management skills and experience in the community rehabilitation setting

  • Demonstrated organization, planning and problem solving skills

  • Highly developed interpersonal skills that incorporates excellent communication

  • Demonstrated commitment to service improvement and professional development

  • Current national criminal history check, or willing to obtain

  • Current Victorian driver’s licence

About the Department
This position is part of the Health Independence Programs (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning.

St Vincent’s Hospital Melbourne (SVHM) HIP consists of integrated hub sites across several locations including in Fitzroy, Kew, North Richmond and Fairfield.

HIP at SVHM is delivered in partnership with North Richmond Community Health (NRCH).

This position is part of the HIP Community Rehabilitation team. Community rehabilitation is currently provided from our centres at Kew and Fairfield.   

The key aim of Community Rehabilitation is to help people who are recovering from traumatic injury or illness, are frail or chronically ill to regain optimal function and to allow people to maximise their independence.

The programs offer a supportive and dynamic team environment which focusses on patient-centred care.
