Job Description Summary

The Telehealth Behavioral Health Care Manager (BHCM) reports to the Collaborative Care Manager in the Center for Telehealth. This position serves as a BHCM for a telehealth-enabled collaborative care program supporting patients in rural primary care sites. The BHCM supports patient enrollment in program, administers regular behavioral health assessments, provides brief intervention, motivational interviewing, and case management as appropriate, all remotely via telehealth. The BHCM is responsible for the coordination of behavioral health treatment with the primary care providers (PCP) at rural sites and proactively communicates patient needs and treatment recommendations to the PCP. The BHCM will also engage in weekly caseload review and consultation with the psychiatric consultant. Additionally, the BHCM will support training and implementation of this program at rural clinic sites and serve as a key member of the pilot grant research team.

Job Description

Requirements: Master’s degree from an accredited college/university required in social work, psychology, counseling, or social welfare program plus 1-2 years of related psychiatric/behavioral health case management experience OR Bachelor’s degree in social work, nursing, psychology, counseling, or social welfare program plus 3-5 years of related psychiatric/behavioral health case management experience required. Clinical licensure preferred. Critical thinking skills and strong interpersonal skills essential. Excellent organizational and time management abilities, and able to work effectively within a multidisciplinary team. Skilled with patient interaction, problem solving, navigation, and listening. This position has the opportunity to work primarily remotely in SC, with occasional travel to primary care sites and the MUSC Center for Telehealth in Charleston.

Job Responsibility:

1. Support enrollment and triage of patients with assistance of telehealth platform and develop care plans with appropriate level of care.

  • Gather patient information and treatment history, and complete accurate and timely patient documentation including intake form, treatment plan and notes.
  • Utilize the telehealth platform registry to track patient progress, engagement, and to identify patients who require increased support, urgent outreach, and/or psychiatric review.
  • Use the telehealth platform to assign digital health content and review content with patients.
  • Provide patients with training and support with telehealth platform as needed.
     

2. Provide brief intervention and facilitate patient engagement in treatment plan through motivational interviewing and continued follow-up.

  • Coordinate additional referral or interventions to address patient treatment and resource needs.
  • Conduct phone or telehealth sessions with patients regularly for the purpose of assessing patient progress and identifying patient needs.
  • Facilitate patient engagement in behavioral health and referrals for social determinants of health through motivational interviewing techniques and continued follow-up.
  • Be familiar with emergency policies and procedures and utilize them to respond appropriately to any patient emergencies or critical incidents.
     

3. Coordinate BH treatment among the multidisciplinary care team through regular communication with PCP and psychiatric consultant.

  • Proactively communicate patient needs and treatment recommendations to the PCP.
  • Identify patients in need of psychiatric case review and implement psychiatric specialist recommendations into patient care plan.
  • Serve as a liaison to clinicians and administrative staff at participating clinics to address workflow, communication, and patient care.
  • Utilize the telehealth platform and EHR to document patient interactions and outcomes, track changes in clinical symptoms, treatment side effects, or treatment complications, document treatment recommendations to be shared within the collaborative care team, and document referrals and subsequent follow-up.
     

4. Provide training and implementation support to rural practices participating in the program.

  • Support training efforts and serve as resource to new sites.
  • Track implementation progress using logs for research and evaluation purposes.
