Fremont Family Medicine is looking for an Advanced Practice Provider (APP) to join their busy practice caring for patients across the generational spectrum.  Practice currently has 7 physicians and 4 Advanced Practice Providers. 

Team-based care is supplemented with social work, behavioral health, pharmacy, care managers, medical assistants working one-to-one with the physicians.  Large practice population located steps from Gerber Hospital with full laboratory, radiology, PT/OT and specialty and surgical services.

Job Summary

In collaboration with the Physicians provides primary health care services for patients. Performs complete screening examinations and routine exams in order to determine a baseline, reflect the nature of the patient visit, diagnose and promote good health. Prescribes medication and administers medical treatment. Provides for the referral of patients to predetermined consultants based upon identified need with consultation of the supervising physician. Arranges for or refers patient to needed services that cannot be provided at the office.

Essential Functions

  • Ability to formulate an individualized plan of care indicated by the patient’s response to treatment, and evaluate overall plan for effectiveness.
  • Collaborates with the physician in order to plan and coordinate patient care.
  • Perform complete screening exams and routine exams in order to determine a baseline, reflect the nature of the patient visit, diagnose.
  • Arranges for ordered diagnostic, surgical and therapeutic services. Arranges for physician consultation/referral appointments.
  • Calls medication refills or new scripts into the required pharmacy under the direction of the providers policy.

Qualifications

  • Required Master's Degree Graduate of an accredited Physician Assistant educational program
  • 1 year of relevant experience office practice setting/previous clinical experience in area of speciality Preferred
  • 6 months of relevant experience Required
  • LIC-Physician Assistant - STATE_MI State of Michigan Upon Hire required
  • CRT-Basic Life Support (BLS) - AHA American Heart Association 60 Days required Or
  • CRT-Basic Life Support (BLS) - ARC American Red Cross 60 Days required
  • CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 60 Days required
