- Assesses bereavement needs and provides bereavement follow-up to each bereaved referred to the Bereavement Program.
- Plans, develops, and implements Bereavement Support Groups and educational workshops on specific grief issues. Plans Hospice Memorial Service(s). Participates in Hospice Team meetings.
- Participates in in-service, staff and other agency meetings for the purpose of planning, coordinating and generally improving the quality of service provided by the agency.
- Participates in the orientation and training of new employees to the Bereavement Program. Develops educational materials and resources for distribution to families and staff.
- Ensures that required documentation for activities within areas of responsibility is completed in an accurate and timely manner.
- Actively participates in safety initiatives and risk mitigating measures where appropriate and completes all position and unit safety related competencies and requirements on a timely basis.
- Required Master's Degree Social Work, Counseling Psychology, Pastoral Care/Divinity or Nursing
- Experience with grief and loss. Required
- LIC-Driver's License - STATE_MI State of Michigan required
