The Stars Population Health Strategy Lead is responsible for developing strategies and executing initiatives to improve health outcomes for defined populations. This role will design evidence-based programs, coordinate care management, and collaborate with internal and external stakeholders to address health disparities, optimize resource utilization, and advance quality initiatives. A broad range of skills including strategic thinking, data interpretation, and program management will be required.

HQS is committed to caring for our customers and delivering high plan quality as rated by the Centers for Medicare and Medicaid Services (CMS).

         

The CMS Stars quality rating system evaluates Medicare Advantage and Prescription Drug Plans using approximately 40 measures covering preventive care screenings, health condition management, health outcomes, patient experience, and plan operations.

The Stars Population Health Strategy Lead is responsible for developing strategies and executing initiatives to improve health outcomes for defined populations. This role will design evidence-based programs, coordinate care management, and collaborate with internal and external stakeholders to address health disparities, optimize resource utilization, and advance quality initiatives.

Key Responsibilities

  • Design and implement population health initiatives that address health risks, disparities, and gaps in care
  • Analyze data and population metrics to identify trends and opportunities for improvement
  • Measure the effectiveness of programs and adjust strategy as needed to achieve targeted outcomes
  • Build and maximize relationships with key stakeholders in member- and provider-facing roles in order to improve our performance among the target populations
  • Responsible for initiative performance monitoring and development of deep, holistic operational reporting, with regular executive summary roll-up to Stars leadership

Required Qualifications

  • 8+ years of strategy development and implementation
  • Proficiency in evaluating complex data, using that data to tell a story, and presenting it clearly
  • Ability to interpret and leverage data and analytics to improve strategy and make recommendations
  • Experience in strategy consulting, business strategy, clinical and/or operations areas
  • Strong presentation skills
  • Demonstrated ability to articulate ideas effectively in both written and oral forms
  • Effective facilitator and influencer

Preferred Qualifications

  • Master's Degree in Public Health, Business Administration or a related field
  • Knowledge and experience in health care environment/managed care and/or Medicare Stars
  • Agile or product management certification a plus
  • Strong analytics background

Work at Home Requirements: To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

 

Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


 

$115,200 - $158,400 per year


 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
