Position Summary

The Analyst, Care Manager is responsible for providing comprehensive care management services to members enrolled in the New York Managed Long Term Care (MLTC) Plan. This role conducts psychosocial assessments, identifies member needs, develops and implements person-centered service plans, coordinates long-term services and supports, and facilitates access to medical, behavioral health, and community resources. The Care Manager collaborates with members, caregivers, providers, and interdisciplinary teams to promote member independence, improve health outcomes, address social determinants of health, and ensure the delivery of quality, cost-effective care.

Required Qualifications

  • Active and unrestricted New York State Social Work license (LMSW or LCSW), as required.

  • Minimum 2 years of experience in care management, case management, managed care, behavioral health, long-term care, home care, or a related healthcare setting.
  • Knowledge of New York Medicaid, Medicare, Managed Long Term Care (MLTC), and Long-Term Services and Supports (LTSS).
  • Experience conducting psychosocial assessments and developing person-centered care plans.
  • Strong communication, care coordination, advocacy, organizational, and problem-solving skills.
  • Ability to work collaboratively with interdisciplinary teams, providers, members, and caregivers.
  • Proficiency in Microsoft Office applications and electronic documentation systems.
  • Ability to travel within the assigned service area to conduct member visits as needed.

Preferred Qualifications

  • Experience working with Managed Long Term Care (MLTC) populations.
  • Knowledge of UAS-NY assessment processes and person-centered service planning.
  • Experience addressing social determinants of health, behavioral health needs, and community resource coordination.
  • Experience working with individuals who have chronic conditions, disabilities, or complex psychosocial needs.
  • Familiarity with New York community-based services, waiver programs, and long-term care resources

Education

  • Master's Degree in Social Work (MSW) from an accredited college or university required.
  • Active New York State LMSW or LCSW licensure preferred or required based on business needs.
  • Equivalent combination of education and relevant care management experience may be considered where applicable.

Pay Range

The typical pay range for this role is:

$21.10 - $44.99

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
