About the Role
As part of the Healthcare for the Homeless (HfH) program, you will assess eligible, high-risk clients living with complex healthcare needs, and provide care coordination that facilitates the client's self-management strategies in order to improve health outcomes and prevent avoidable hospital presentations. You will support the ALERT team and the Cottage, which is a 5- bed health facility in a homely terrace house setting, providing personal and nursing care for men and women experiencing homelessness.

• Maximise a patient's functional independence through assessment and treatment planning, implementation and evaluation

• Provide appropriate discipline specific clinical management, discharge planning and community liaison for all relevant patients

• Provide clinical and practical therapeutic support for Cottage patients

• Provide discipline and care coordination clinical expertise and leadership in clinical/team meetings

The classification range for this position is Allied Health Grade 2 ($88,467 - $101,691 per annum, pro rata) or Registered Nurse Grade 4A YW7 ($107,962 per annum per annum, pro rata).

Your Contribution
• Minimum 3 years nursing or relevant allied health experience in an acute setting
• Eligible for registration with AHPRA/professional organisation

• Ability to engage with clients and assist them to identify their health and wellbeing needs, and coordinate services and providers to achieve desired outcomes

• Ability to work autonomously and as part of an interdisciplinary team

• Ability to effectively manage and negotiate solutions to complex problems
• Current National criminal history check, or willing to obtain

HfH aims to integrate and better coordinate existing services within St Vincent’s Hospital and where service gaps are identified, support the development of new services to meet the needs of our clients.
