Job Description:

Position Title: Chronic Disease Nurse Practitioner / Transitional Nurse Practitioner
Employment Type: Maximum Term Full Time - 30/06/25
Hours per week: 38 hours per week
Position Grade: Nurse Practitioner Level 1-4 / Transitional Nurse Practitioner (CNC Grade 2)
Salary: $135,591.23 - $145,181.65
Base salary per annum)

About the role:
We have an exciting opportunity to work in a dynamic team leading our approach to the provision of quality care to clients of the Chronic and Complex Care team.

The Chronic Disease Nurse Practitioner is a highly skilled nurse clinician who delivers evidence-based, independent clinical care to patients following established guidelines for advanced nursing practice. The Chronic Disease NP/TNP will autonomously and collaboratively oversee the care of patients with chronic conditions like Palliative Care, Hematology/Oncology, Motor Neuron Diseases, COPD, Transplant, or other life-limiting illnesses within the local community served by St Vincent’s Hospital.

With a wealth of experience in chronic disease management, the Chronic Disease NP/TNP will offer expert assessment and treatment using advanced nursing expertise. They will also take on a leadership role in the community, providing both clinical and professional guidance.

In addition, the Chronic Disease NP/TNP will perform thorough patient evaluations and manage various chronic disease-related conditions in community settings, including individuals in Residential Aged Care Facilities (RACF’s) and those in Homeless Health environments.

About you:
We are seeking a skilled and energetic Nurse Practitioner/Transitional Nurse Practitioner who possesses a passion for person centred care.

Selection Criteria:
1. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries

2. Currently registered as a registered Nurse with the Australian Health Professionals Regulation Agency (APHRA) and endorsed by the Nursing & Midwifery Board of Australia (NMBA) to practice as a Nurse Practitioner [or undertaking study that will lead to endorsement as a nurse practitioner by the NMBA], and current unrestricted NSW Class C Driver’s License.

3. Demonstrated high level leadership within the workplace including the ability to form productive working relationships through highly developed communication, negotiation, facilitation and conflict resolution skills

4. Comprehensive understanding of clinical governance, clinical practice development and current nursing issues

5. Ability to deliver, evidenced based, patient centred care for within a complex environment, autonomously and collaboratively, using advanced and extended clinical skills

6. Demonstrated experience in quality improvement principles, clinical policy and procedure development and implementation

7. Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice

8. Demonstrated commitment to own professional development and support of others professional development through leadership, delivery of education and clinical practice support/supervision.
