- Maintains and ensures that all charted progress notes are informative and descriptive of the services provided and of the resident’s response to the service.
- Using various assessment tools assists in meeting the physical, mental and psychosocial needs of residents and family members.
- Assist residents and family members with programs, transportation, financial assistance, community social health and referrals to appropriate social service agencies and treatment programs.
- Interviews residents and family members to assist in making plans for treatment goals, care plans and discharge planning.
- Interprets social, psychological and emotional needs of the resident and family member to resident team care members, attending physician and ancillary team staff. Attends resident’s care conferences.
- Exhibits excellent customer service skills and behaviors toward internal and external customers and co-workers.
- Adheres to department customer service, service excellence, and performance standards.
- Demonstrates personal commitment to the Organization’s core values through active involvement in the performance improvement process.

- Required Associate's Degree Minimum of an associate’s degree in social work in a human services field including, but not limited to sociology, special education, rehabilitation, counseling and psychology.
