Completes outreach, assessment, triage and education duties with community members to ensure access to appropriate services for individuals needed mental health assistance as a diversionary process prior to commitment. After inpatient treatment create community connections for services for continued success in the local community.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Bachelor’s degree from an accredited college or university in a human service or business related field

2. West Virginia state and Federal criminal background check required for DHHR WV CARES regulated area.

PREFERRED QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

EXPERIENCE:

1. Two years of post-degree position specific experience

2. Experience in other community based social work settings

1. Formulate, direct and coordinate informational activities and policies to promote education and services within the community for purposes of early intervention

2. Coordinate with local agencies and organizations for appropriate referral mechanisms and coordination of services to the population

3. Identify individuals needing services prior to initiation of inpatient or commitment process to ensure access prior to crisis events

4. Clinically assess individuals for service needs and assist in gaining access to treatment and  needed services - including  development of housing, funding mechanisms, and support networks

5. Locate and contact organizations- such as local law enforcement, hospitals, Crisis Units to offer education and services for diversion and post treatment community integration

6. Ensure individuals are connected with services to provide stability in the community

7. Advocate on behalf of consumer needs for treatment, services and integration

8. Coordinate reintegration of mental health consumers into their local community

SKILLS AND ABILITIES:

1. Ability to read, analyze, and interpret common scientific and technical, financial reports, and legal documents

2. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community

3. Ability to write speeches and articles for publications that conform to prescribe style and format.

4. Ability to effectively present information to top management, public groups, and/or boards of directors.

5. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations

6. Ability to define problems, collects data, establish facts, and draw valid conclusions

7. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables
