Job Description:

Position Title: Registered Nurse - Transitional Aged Care Program
Employment Type: Maximum Term Full Time
Hours per week: 38 hours per week
Position Grade: Registered Nurse, Years 1 to 8
Salary: $70,0049.73 - $98,351.39 Base salary per annum
Enterprise Agreement: The Named NSW (Non-Declared) Affiliated Health Organisations' Nurses Enterprise Agreement 2017

About the role:
We are thrilled to offer an amazing chance to join our vibrant and expanding team, where you will play a leading role in delivering top-notch care to clients enrolled in the Transitional Aged Care Program. As a registered nurse in this role, you will be responsible for providing nursing care to patients referred to the Transitional Aged Care Program (TACP). Working alongside a dedicated and goal-driven multidisciplinary team, you will conduct thorough assessments, develop discharge plans, and implement therapeutic interventions tailored to each individual's needs. Additionally, you will also be involved in case management for elderly clients in their homes after their hospital stay. This position offers a Monday to Friday schedule, with an on-call requirement on a rotating roster. About you:

Looking for a talented and dynamic Registered Nurse with a dedication to person-centered care. We encourage applications from individuals who are enthusiastic about delivering top-notch care to older people in the community, adept at communication, team-oriented, and possess strong time management abilities.

You will manage your day out in the community, providing nursing care and case management to achieve independence goals for older people on the transitional age care program. You will be part of our multidisciplinary team to provide rehabilitation in an 8 week program.

Selection Criteria:

1. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia

2. Currently registered as a Registered Nurse (Division 1) Australian Health Professionals Regulation Agency

3. Valid Class C Driver's License or above

4. Demonstrated effective communication and interpersonal skills

5. Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice as a registered nurse

6. Demonstrated effective time management skills and ability to prioritise competing workloads within a complex environment
