Position Overview:

This position has delegated responsibility for adhering to Hospital and Nursing Service policies, procedures and practices in the provision of safe, thorough bedside care of patients; cooperative, efficient work practices; and the performance of technical, manual skills delegated by and under the supervision of a professional registered nurse.

Job Description:

  • Carries out the plan of care within the scope of the LVN Practice Act as it relates to the patient's physical needs which includes but is not limited to:
  • Observation
  • Observes, documents, and informs Registered Nurse of changes in patient's physical and psychosocial conditions;
  • Planning
  • Assists in the development and maintenance of a formal written plan of care for patients;
  • Intervention
  • Carries out the plan of care. Implements timely interventions and includes use of CareMAPs, and policies and practices.
  • Evaluation
  • Evaluates nursing care within the scope of the LVN Practice Act provided on assigned patients under the supervision of the Registered Nurse.
  • Administers oral and parenteral medications and IV fluids and additives within the scope of the LVN Practice Act.
  • Checks, transcribes and "signs-off" physician orders as permitted under the LVN scope of practice.
  • Carries out the plan of care as it relates to the patient's and family's emotional and educational needs. Participates in reports, multidisciplinary rounds, patient care conferences, team/unit staff meetings, and educational programs. Reports observations to assist in the evaluation of the plan of care. Participates in patient/family education as directed.
  • Assists in the development and implementation of the discharge plan with the Registered Nurse, patient, and family.
  • Sets priorities and initiates emergency interventions.
  • Communicates, and coordinates with all members of the patient care team in planning care, observing effectiveness, teaching, monitoring progress and discharge planning.
  • Documents patient care data using computerized patient information system or on non-computerized record.
  • Communicates with patient and family in a clear, open, and collaborative manner showing care and concern.
  • Assists in identifying and observing patients at risk for untoward events.
  • Participates in review of patient care center outcomes: clinical, customer satisfaction, operational and cost.
  • Provides for a safe working environment for the Nursing Unit by having knowledge of relevant policies and practices. Reports and corrects unsafe conditions.
  • Supports the mission, philosophy and goals of the Medical Center.
  • Contributes to and actively participates in the quality assurance and quality improvement processes, per JCAHO and/or other regulatory agencies.
  • Supports the development of unit/team goals, and attends team staff meetings.
  • Demonstrates financial awareness by responsible use of equipment and supplies to control costs. Provides input into resource allocation and the budgetary process.


EDUCATION

  • Equivalent experience will be accepted in lieu of the required degree or diploma.
  • Other: Will consider new graduate LVN if graduated within the last year.


CERTIFICATION & LICENSURE

  • LVN-Licensed Vocational Nurse
  • LVNIVBWP-Vocational Nurse Intravenous Therapy Blood Withdrawal Provider
  • BLS-Basic Life Support Healthcare Provider


PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:

  • Minimum one year experience within the last two years as a LVN in an acute care setting or sub-acute setting required.
  • Specialty experience preferred for each Patient Care Center (ie., Women’s Services, Subacute, Rehabilitation, Transitional Rehabilitation, etc.).


SKILLS AND KNOWLEDGE

  • Able to communicate clearly and effectively in English.
  • Able to write legibly and effectively present ideas, document activities, read and interpret written information.

Pay Range is $38.14 to $45.59 / hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
