CANCER CARE COORDINATOR

About the Opportunity:
We are currently looking for a permanent part time 0.8FTE Cancer Care Coordinator to be the central point of contact for patient, staff and family queries relating to our cancer and palliative care services at St Vincent’s Private Hospital Northside. Reporting to the Oncology Nurse Manager, you will provide clinical practice, an effective communication link between all stakeholders, and genuine care consistent with the SVHA mission and values of compassion, justice, integrity and excellence.
The role entails:
Effectively coordinating the continuum of care within the cancer and palliative care service, including the provision of support with navigating the health care system (e.g. assisting with booking appointments, streamlining investigations, explaining procedures and advocating where appropriate)
Acting as the key liaison between patients, their families and members of the health care team
Coordinating and participating in multidisciplinary meetings
Ensuring the cancer and palliative care nursing standards are consistently applied cross all service areas
Provision of clinical practice for cancer and palliative care patients as required
Assessment and screening for clinical and supportive care needs and of patients at risk of adverse clinical or psychological outcomes
Facilitation of physical, emotional and educational support to patients and families
Ensuring appropriate referrals to specialists, allied health and supportive services are promptly managed
Participation in and support of educational activities and contribution to the development of self and team members
Data management and collection.
To be successful you will have:
Recent experience (within the last 6 months) working at a senior level in a cancer and/or palliative care setting within an acute facility
Excellent interpersonal, written and verbal communication skills
A warm and engaging demeanour, with the ability to influence stakeholders
Experience providing leadership and coordination of the patient care continuum
Ability to coordinate and multitask in a fast paced environment
Excellent patient centred care skills
Sound computer skills
