About the Role
Under the guidance and support of the Multidisciplinary Team, you will deliver exceptional clinical treatment and support to individuals with a lived experience of mental illness.  Managing a case load, your role extends beyond clinical responsibilities to actively cultivate and sustain collaborative partnerships with various internal and external service providers.

  • Implement the Strengths Model to empower individuals in their recovery journey, emphasizing their capabilities and resilience.
  • Collaborate seamlessly within the multi-disciplinary team, supporting other care partners and ensuring a holistic approach to patient care.
  • Uphold meticulous documentation practices in line with local policies, encompassing clinical records, outcome measures, and Client Management Interface reporting.

Classifications for this position will range from:

  • OT/SW Grade 2 YB20 - YB23 or YC42 - YC45 ($86,304 - $99,210 per annum)
  • Psychologist Grade 2 PK1 - PK4 ($90,812 - $102,757 per annum)

Your Contribution

  • Experience in the provision of assessment, discipline specific intervention and case management services for people with serious mental illness and complex needs and their
  • Commitment to work collaboratively and effectively as part of the multidisciplinary team with multiple key stakeholders both internal and external of the St Vincent’s Health organization (including consumers, carers, family members and community agencies)
  • Highly developed interpersonal and communication skills, including problem solving ability and conflict resolution skills, as well as sound oral and writing skills.

About Hawthorn Continuing Care Team
The Continuing Care Team provides specialised treatment and ongoing support throughout the week during business hours. Employing the Strengths Model of Recovery-Oriented Practice, the multidisciplinary team delivers tailored clinical and therapeutic interventions to meet the specific needs of consumers with a lived experience of mental illness.
