Job Description Summary:

JOB SUMMARY
In accordance with OhioHealth’s mission and values, the System Pharmacy Medication Utilization Coordinator acts under the direction and support of the System Pharmacy Medication Utilization Advisor to promote rational, evidence-based, clinically appropriate, safe, and cost-effective medication therapy.

MINIMUM QUALIFICATIONS
Doctor of Pharmacy Degree
RPH - Registered Pharmacist
Field of Study: Pharmacology

SPECIALIZED KNOWLEDGE
Pharmacy; Registered Pharmacist in the State of Ohio; Advanced degree or equivalent Experience. Knowledge of business, and or clinical pharmacy health care environment trends. Preference given to those with residency experience. PGY-2 residency (HSPA, Managed Care, Informatics, Medication Use Safety, Drug Information, Medication Systems, Medication Management, or other) or 3 years hospital pharmacy experience is preferred.

INFORMATION SECURITY
Maintains confidentiality of log-on password(s) and security of other authentication devices (e.g., key fobs, proximity devices, etc.).

Adheres to all information privacy and security policies, procedures, standards, and guidelines.
Promptly reports information security incidents to the OhioHealth Information Security Officer.

RESPONSIBILITIES AND DUTIES

- Supports the Pharmacy & Therapeutics (P&T) Committee decision-making process.

- Develops and coordinates project plans involving Medication Use and Formulary Management and provides high quality deliverables in a timely manner.

- Promotes and facilitates cross campus multidisciplinary collaboration to achieve effective system-wide decisions.

- Coordinates the vetting, approval, and implementation of medication utilization updates (i.e., formulary restrictions, therapeutic interchanges, medication use guidelines/policies, etc.).

- Manages the P&T guideline every 3-year review process.

- Integrates Medication Use and Formulary Management improvement initiatives, activities and changes with clinical and operational pharmacy programs and committees.

- Maintains awareness of the non-oncology pharmaceutical pipeline to proactively address medications that may need added to the acute care formulary.

- Coordinates the vetting, approval, and release of changes to the OhioHealth Intravenous Administration Handbook.

- Acts as the OhioHealth Lexicomp Information Management System Administrator.

- Acts as the Medication Use liaison to work with the designated pharmacist informaticist on implementation of P&T changes into CareConnect.

- Other duties as assigned.
