United Health Services is seeking a Virtual Health- Hospital Care at Home Physician (IC) to join our Community Based Clinical Services 04 department at NY in Johnson City, NY. This position is benefits eligible.

Virtual Health- Hospital Care at Home Physician (IC) / Overview


 

UHS is seeking a board certified/board eligible Family Medicine physician for our Virtual Health department to participate in our Hospital Care at Home program. The current patient panel consists of patients discharged early from inpatient care and emergency departments who may be at substantial risk for re-admission. The future goal is to provide acute care / inpatient level of care to patients in their homes as part of a team-based model of care which includes home visits from RN and /or paramedics. This physician will also cross-train in providing care in the UHS Virtual Walk-In. This physician can either work remotely or in a hybrid model.
Must be board certified/eligible.
